Senior Outreach Nurse Program

What is the Senior Outreach Nurse Program and how can it help?

Outreach nurses receive referrals to visit seniors from a variety of sources, such as: self-referral, family members, neighbors, physicians, and other service providers. 
The program's goal is to support seniors so they can live independently as long as possible and avoid unwanted or premature placement in healthcare facilities. 
Nurses work with clients and other care providers to complete a thorough assessment and develop a plan of care. Follow-up visits are provided every six months, or as necessary, to respond to the changing needs of the clients. 

Who is eligible for the Senior Outreach Nurse Program?

Senior 65 years and older who live in Sierra County. Everyone in this age group qualifies for our services, regardless of income. 

What does it cost?

This service is FREE of charge through funding from Sierra County Public Health.

What can the Senior Outreach Nurse do?

  • Provide in-home assessments to review a client's medical, social, and environmental needs.
  • Provide health education consistent with the client's needs. 
  • Discuss referrals and resources. Help link them to those services as needed.
  • Review medications and set up programs to help ensure their safe administration in collaboration with medical care providers.
  • Provide follow-up as needed.
  •  Conduct blood pressure and health screening at various locations in the community.
